General Summary of Duties

***This position is eligible for a one-time $5,000 sign-on incentive bonus***


Provides health services, care management and care coordination to consumers through pre-evaluation, assessment, planning, education, intervention, and review. Participates as part of an inter-disciplinary team.


**This position will work with the ACT team and on-call responsibilities with compensation are included in this position**


Salary Range: $47,008 - $63,960 per year

Education/Experience Requirements

Minimum of an Associate’s Degree in Nursing and a Registered Nurse license in the state of Michigan is required. Experience working with individuals with mental illness or intellectual/developmental disabilities is preferred. Valid driver's license required. CPR certification preferred.
